
Did you know that more than 90% of Assist ME’s referrals come directly from the medical community? By strengthening provider knowledge of our program and streamlining referrals, Assist ME works alongside the medical community to bridge health-equity gaps for uninsured and under-insured neighbors across the greater Charlotte region, including Medicaid gaps.
By year-end 2025, nearly 250 individual referral partners—including rehabilitation facilities, physical and occupational therapists, social workers, hospital discharge teams, home health providers, physicians, nurses, and hospice staff—were actively connecting patients to our services. Kudos to our referral partners! With your help, we provided over 3000 items to 1900 individuals last year alone.
While our network grows every month through word of mouth, social media, health partner fairs, and direct requests from providers or organizations who want their teams trained on how to access free mobility equipment for patients in need. As mobility access continues to be one of the most overlooked social drivers of health, properly fitted equipment helps patients heal or manage their conditions safely — reducing fall risk, enabling them to attend follow-up visits and physical therapy, and supporting their ability to rejoin their communities with dignity.

To support the growing need for mobility equipment to our most vulnerable neighbors, Assist ME offers a streamlined training session for medical providers and support staff that is available in-person for groups of 10 or more or as a virtual Lunch & Learn for both small and large teams. Course content is the same and takes about 30–40 minutes plus Q&A.
Learning Objectives
Each session provides a clear, practical walk-through of how to refer patients for free mobility equipment through the MOVE (Mobility, Opportunity & Vital Equipment) Program.
Participants will:
- Understand Assist ME’s mission and how it supports patient care
- Identify which patients qualify for free mobility equipment
- Master the MOVE Program referral and application process
- Understand the equipment fulfillment and patient pickup process
- Learn who to contact for questions or support
